About the Initiative

A Collaborative Approach to Wellbeing

Led by the Australian Social Prescribing Institute of Research & Education (ASPIRE), this project brings together national and international experts to design, implement, and evaluate social prescribing in Queensland. Our approach is evidence-based, community-led, and focused on long-term impact.

Why Social Prescribing?

Social prescribing links individuals to local services, activities, and support networks to improve health and wellbeing. By fostering connections and addressing social determinants of health, we empower communities to take charge of their own wellbeing.

Our Goals

We are developing a flexible, scalable toolkit that Neighbourhood Centres in Queensland, councils, and health networks can use to create social prescribing programs tailored to their communities. Our goal is to make social prescribing accessible, impactful, and sustainable.

How it Works?

Our social prescribing approach follows a collaborative five-step process designed specifically for Queensland communities. First, we identify local needs and priorities by analysing data and community insights. Next, we map existing community strengths and assets, uncovering valuable opportunities for meaningful connections. Working closely with local families and partners, we then co-design personalised solutions tailored to each community's context. Specially trained Link Workers facilitate these connections, supporting families directly and removing practical or financial barriers to participation. Finally, we rigorously evaluate outcomes and continually refine our approach, ensuring long-term, sustainable impact for children, families, and the broader community.
